
Working Papers
Instructions for Employment Certification (Working Papers):
Students must obtain an employment certificate ("working papers") if they intend to work.
Students should see Ms. Fox, the Parent Coordinator, for working papers
You Need To Bring in the Following Documents for Working Papers:

The "Application for Employment Certificate AT-17 form" with Section I completed and SIGNED by a parent/guardian. This form can also be obtained on this website (see below) as well as in Ms. Fox's Office.

Evidence of Age in the form of: a birth certificate, a passport, OR a State ID.

An original doctor's note (stating the minor is "fit for employment/physically fit to work") or an original certificate of physical fitness completed by a physician.
YOU NEED TO BRING IN ALL OF THESE DOCUMENTS TO HAVE WORKING PAPERS ISSUED.
Once we have all of the documents, it will take up to a day to process, so the earlier you bring in these documents, the better - especially if you need to meet a deadline!
